Ordering Information

To request TLD for one or more of our services, please complete the appropriate form from the options below and email it to RDS@MDAnderson.org. Service may be scheduled for checks to be done monthly, quarterly, semi-annually, annually or by request.

Payment Information

Payments are due upon receipt of invoice. Please include a copy of the invoice with payment for accurate posting to your account. Payments can be made by check or money order payable to: MD Anderson Cancer Center, RDS # 600507/30/116572/43019/46, or by credit card. If your institution requires a purchase order for payment, please provide this to us at the time of order or prior to shipment.
