Physicians and pathologists can request consultations and specialized laboratory testing from the Department of Hematopathology. Outside services in Hematopathology handles the accession of all referral and consultation material submitted. These samples include blood, bone marrow, tissue, slides and blocks.

Diagnostic Testing
For diagnostic consultations only, please complete the Hematopathology Consultation Request form. Both glass slides and a representative paraffin block are encouraged. (Consultation requests consisting of slides and blocks only should be sent to R4.2137C.)
Laboratory Testing
For laboratory testing of samples, please complete Hematopathology Outside Services Test Requisition. Send the the form along with the samples to the appropriate lab; the address is indicated on the instruction form attached to the requisition.
