Services and Fees
The RQA Lab’s fee for the phantom service includes round-trip shipping for phantoms mailed within the United States. Shipping charges will be added to the fee for phantoms mailed to all other countries.
You may call to inquire about an estimate of shipping charges to your location.

Billing Information
Fees may be paid with a company purchase order (PO#), credit card or wire transfer. Invoices will be sent by General Accounting to the contact person at the billing address provided on the Phantom for Fee Request Form.
Checks must be made payable in US dollars to MD Anderson Cancer Center.
